Before your appointment, please complete all intake forms and upload a current list of your medications and supplements, or bring them with you to your visit. If you have recent laboratory results or imaging reports that have not yet been uploaded to your patient portal, please bring copies with you. We also ask that you upload a photo ID and copies of your current health insurance card(s) prior to your appointment.

During your initial visit, Nicole will take time to understand your health history, current concerns, and wellness goals.

Based on that discussion, personalized recommendations will be provided, and any previous laboratory testing or imaging will be reviewed.

Lab orders and/or testing kits will be provided before the end of your appointment so you can complete testing prior to your Lab Review visit.

You will have access to our HIPAA compliant patient portal. We do our best to communicate with our patients through here. We ask all patients to complete their portal registration at least 48hrs before their appointment. This allows us to communicate with you privately and easily.

This follow-up visit is designed to review the labs and/or testing completed after your initial consultation. We will discuss your results and review your current supplements and medications.

Based on your results and individual needs, we may recommend adjusting, discontinuing, or adding supplements and/or medications. Your personalized recommendations and protocol will be reviewed with you during the appointment and provided to you by the end of your visit.

You will leave your first follow up appointment with a personalized care plan in place.

Ongoing follow-up appointments will be tailored to your individual goals, progress, and treatment plan to ensure you receive the support and guidance you need along the way.
